Training Report Writing Methodologies


This is a little scope of report i used to present to the board of directors and the executive director of the organization that aims about good health.
Opening prayer by the M&E officer from ECI at about 10:20am. It was followed by a general introduction by all participants. The participants were drawn from the four CSOs working in the SHiPS for MARPs project in Akwa Ibom State, HQ M&E, State STL, SPO, M&E officer and SFH MIS/Referral officer.
Objectives of the Training.
This was stated by Dr. Richard – M&E officer for SFH (HQ)
·        To train all M&E project officers on the use of DHIS2 for data capturing and generating of report
·        To define the modality for successful referral of identified clients
·        To support all the CSOs M&E to enter backlog of data from Jan to Aug
Expectations at the end of the training
·        All CSO M&E officers should be able to understand and use DHIS2 tool efficiently.
·        Understand the timeline on sending reports.

PRE TEST was given for 15min followed by tea break
Data Quality Assessment 2016 result
During the training, time line is important in data quality. Time line is to inform decision, help in future programming. Also DQA entails where resources should be channeled to for effective implementation.
The 6 DQA
·        Accuracy or validity: All data must be original and accurate.
·        Reliability: This require the tools used in capturing the data. There is a standard tool used.
·        Timeliness: This means time for submission of data for future programming.
·        Completeness: This means up to date data obtained between a certain period of time
·        Integrity: Quality of data obtained or trust
DQA is carried out after six month on the commencement of the project. It must be carried out using required tool with necessary indicators.
CSOs will be reporting on the DHIS2 on the MPPI while state will concentrate on the treatment of clients on OSS and DiCs.
Intensive phase is now 3 months and no more 4. A peer is reached with 6 contacts henceforth within 3 months. In the BM sites, a contact is only once a month.
 Counting for one service is once a year e.g HTS and STI services.
Challenges on HTS were shared during the training as follows;
·        Counting of no. of test instead of unique counts
·        Incomplete filling of tools e.g no dates, location etc.
·        Failure to summarize result by HTS testers
We were made to know that testing of clients of FSW who is positive can be tested as well
Snow Ball Clients are those clients identified by a partner of FSW.
From October 1, 2016 referral to other facility is not valid in the project and every clients tracked by other facilities will not be counted anymore except the client is transferred with a transfer letter to OSS.
Any FSW screened and identified positive for STI must be tested for HIV.
Dr. Richard said that when conducting trainings, the following points must be taken note of.
·        Title of training
·        Counting of staff attendance instead of trainees
·        Missing one day of a training nullify you as been trained
·        Pre and post test not rendered.
N/B all original documents must be with the CSOs. EDs must ascertain (sign) to the data reported by the M&E officer before the data in entered into the DHIS2 software.

DATA FLOW AND TOOLS FOR CAPTURING DATA
PE/HTS officer are the important data collector
6 PEs submit to CF
5 CF submit to CS
CS submit to CSO M&E monthly who will collate the data by LGAs
M&E conducts internal validation and is approved by ED then the data is entered into DHIS2

ACTION POINT
When collating data make an analysis on the location of testing (LGA), no. of tests, no. of +ve etc. Use bar chat to analyzed result.

DHIS2 PROPER
M&E officers were properly guided on the use of DHIS, its different platform as well as its indicators.
Under “other Indicator” Sheet every condom distributed that is not of target group should be reported as “pick up at CSO offices”
Also in gender, the number reached with MPPI should be reported same.

FIELD SUPERVISION AND ROLE OF M&E OFFICERS
Ø PO ensures that the project is implemented according to strategy and that all services provided are documented using the data capturing tool.
Ø M&E review the data capturing tools to ensure.
Ø Conduct joint visit to the field to monitor implementation of activities
Ø M&E ensures that all tools required by program are available
Ø At the end of the month, M&E collate data on project activities and summarizes the data on the monthly summary tool.
Ø M&E analyze the data to describe the result for the month.

Record keeping.
M&E ensure filing of project activities.
Each peer educator should have a file and activities filed by months
All HTS officer must have a file documenting monthly activities
Condom and lubrica collection and distribution are documented and filed.

Data Security
Ø All original hard copy of documents must be kept in a secure place not accessible to everyone.
Ø Photocopy should be sent to state office as backup.
Ø M&E should report to state government monthly
Ø Use national tool in reporting

DHIS2 OVERVIEW
It is a data collection tool used for reporting. It is approved by Nigerian govt. it is a web based and runs on major browsers like internet explorer.
Functions of DHIS was given as; Support planning, organization and management

*    Where dimension: this is the location
*    What dimension: this refer to dataset/program or specific intervention
*    When dimension: this is the period or time at which the activity takes place

DAY 2 @ SFH OFFICE
This session was solely for the M&E officers implementing SHiPS project in Akwa Ibom State.
Opening prayer by Nsikanabasi from CYEN. It was followed by recap of day one activities anchored by Mr. Taiwo. However, every participant was asked what they learn on day one and they respond accordingly.
Dr. Richard made the participants to understand that from now henceforth session will be for three months and that any peer that meet a minimum of three contact session within two months interval must be graduated to BM
Tea break was observed at about 11:15am
M&E officers were guided on data entry till the end of day 2.

DAY 3 @ EMMJM HOTELS
Day 3 was all about entering of backlog of data into DHIS2 by individual CSOs.
My observations during the training:
The training was a stepping stone for me as I acquired more ideas basically been expose to DHIS2 software for data capturing and entering. However the following challenges hinders the smooth running of the training;
·        Difficulty in accessing internet services

·        Change of venues and accommodation
